Sign on Incentive of $1,000 Available – $200 payable after successful completion of 90 days, $300 payable after completion of certification or after successful completion of 6 months and $500 after successful completion of one year.
The Family Coach meets program and client needs by managing program referrals, assuring a high quality of care/service to program clients/families, provides for ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual client and family needs specific to building self-sufficiency in families. The Family Coach works with the Community Services Director to assist in reporting functions, including state and federal mechanisms and databases for tracking of services to families. Responsibilities include, but are not limited to: promotional work for the program, assessing needs of the clients and families served; assuring compliance with program requirements, including reporting requirements; assure and promote coordination with internal and external providers, referral sources and community partners; and provides direct care and assessment for program participants. The Family Coach’s overall goal of the position is to provide individual/family case management services to low-income residents including assessments, plan development(s), advocacy, advising, support and service coordination and follow-up. enhance the caliber of client management and satisfaction, in order to improve their quality of life.
